On April 22 we had a pipe elbow failure in our Emergency Department (ED) causing a significant water leak. Crews are assessing the impact and we are in the process of water abatement. Our ED is open, but several rooms are currently offline which could increase wait times. Some surgeries are also being rescheduled.
Consent to Treatment
When you check in at a VMC Clinic or register for hospital care, you will be presented with one of the consent forms below:
- Consent to Treatment at a VMC Clinic (Form 96-23373)
- Consent to Treatment at a Valley Medical Center's Hospital (Form 20-1193)
- Permission to Treat a Minor Without a Parent or Guardian Present (Form 86-0340)
